Hi, If you would like to be considered as a potential home for one of our puppies please fill in the following questionnaire and submit it to us.
After receiving it I will e-mail you with more details.
If you don't hear from me after submitting your questionnaire, please write or call.

If you wish to fax the form you will need to e-mail me to set up a time to fax when I am offline.

Please understand, this is NOT a form to purchase a puppy, this form is only to notify us that you would be interested in being considered as a prospective home, and helps us determine the suitability for puppy placement.
